After that, you\u2019ll need to enter your National Identification Number<\/b><\/span>, which is your government-issued ID number. Type the number without punctuation or symbols<\/b><\/span>.<\/p>

The final two fields ask whether you have a U.S. Social Security Number<\/b><\/span> or a U.S. Taxpayer Identification Number<\/b><\/span>. Since you are applying for a C1\/D visa as a non-U.S. citizen, these should not apply. Enter your phone numbers<\/b><\/span>, just in case someone from the embassy or consulate needs to contact you, though it\u2019s very rare. Most of the communication regarding your visa status will be done through email<\/span>, so make sure to enter a valid and correct email address<\/b><\/span>.<\/p>

For the Passport Book Number<\/b><\/span>, most applicants should select Does Not Apply<\/b><\/span>. Only fill it in if your passport specifically lists a book number.<\/p>

Next, select the country that issued<\/b><\/span> your passport. If your passport was reissued while you were abroad, select the country where it was reissued. Otherwise, select your home country<\/b><\/span>.<\/p>

Enter the Issue Date<\/b><\/span> and Expiration Date<\/b><\/span>, both of which are printed on your passport.<\/p>

The final question on this page asks whether your passport has ever been lost or stolen<\/b><\/span>. If it hasn\u2019t, simply select NO<\/b><\/span> and proceed. If it has, you\u2019ll be prompted to enter additional details such as the old passport number and relevant information.<\/p>

As explained earlier, this tutorial is specifically for seafarers<\/b><\/span> applying for the C1\/D visa<\/b><\/span>, not<\/b><\/span> for the one labeled D \u2013 CREWMEMBER<\/b><\/span> only. Do not<\/b><\/span> select the D visa by itself, or your application will likely be rejected.<\/p>
